Easing Transitions - Preparing Teens for the Adult Health Care Setting

According to the American Academy of Pediatrics, more than half of American youths with chronic conditions, such as childhood cancer or cystic fibrosis, do not receive the support they need to transition from the nurturing pediatric care setting into the increasingly complex adult health care environment. During this time, they face challenges like losing access to their pediatric doctors and care teams and changes to their insurance.

Erika Jewell, LCSW was hired to build the Transition of Care Program at Miller Children’s in 2012. She has more than 18 years of experience as a social worker working with children, adolescents and adults with health problems in many settings. Erika has always liked working with teens and has worked at Miller Children’s for more than 12 years.
